This study explores a wide range of scientific and care provider issues relating to health outcome assessment in primary and hospital out-patient care. In parallel, more fundamental concepts concerning the principles of outcome measurement in chronic disease are addressed, and the limitations and constraints arising from the methods and contexts in which measures have been developed and applied are discussed. Contributions have been drawn from a wide range of disciplines and from researchers and practitioners of outcome assessment across the United Kingdom and mainland Europe.
