From its introduction, oncological chemotherapy has been encumbered by poor selectivity because antiproliferative drugs are often toxic not only to tumor cells but also to important populations of the body’s non-neoplastic cells. Modern targeted therapies interact with defined molecules present on cancer cells, adding increased selectivity to their toxic effects. This book presents an integrated critical view on the theories, mechanisms, problems and pitfalls of the targeted therapy approach.

Until the early 1960s, cancer treatment consisted primarily of surgery and radiation therapy. Most practitioners then viewed the treatment of terminally ill cancer patients with heroic courses of chemotherapy as highly questionable. The randomized clinical trials that today sustain modern oncology were relatively rare and prompted stiff opposition from physicians, who were loath to assign patients randomly to competing treatments. Yet today these trials form the basis of medical oncology. How did such a spectacular change occur? How did medical oncology pivot from a nonentity and, in some regards, a reviled practice to the central position it now occupies in modern medicine?             In Cancer on Trial Peter Keating and Alberto Cambrosio explore how practitioners established a new style of practice, at the center of which lies the cancer clinical trial. Far from mere testing devices, these trials have become full-fledged experiments that have redefined the practices of clinicians, statisticians, and biologists. In this volume are collected 30 papers, 9 round table discus­ sions and 11 communications presented at the ASI Course on "The use of human cells for the evaluation of risk from physical and chemical agents", sponsored by NATO and organized by ENEA. The aim of the Course was to present different scientific ap­ proaches and technical advices in order to get dose-effect relation­ ships which are the basis for risk evaluation. The scientific back­ ground which is behind this approach was extensively discussed. Emphasis has been given to the use of human cells or human data in order to attempt to have a correct and realistic evaluation of the damage in humans. There are many criticisms on the use of animal data for human risk evaluation because of differences between species and between strains within the same species: differences in metabolism, activa­ tion processes and DNA repair ability makes uncertain the extrapola­ tion of animal data to humans. Also data obtained using specific strains or highly inbred strains in order to reduce the variance are not applicable due to the heterogeneity of the human population connected with individual responses. In this respect only the use of human cells enable us to detect the individual variability and to identify sensitive subpopu­ lations that would be at greater risk.
